CMS ACCESS Chronic Care Program

ETN Medical is a participating organization in the CMS ACCESS (Advancing Chronic Care with Effective, Scalable Solutions) Model. We help Original Medicare beneficiaries manage chronic conditions with ongoing support between visits with remote monitoring, care coordination, and close follow-up with your care team.

ACCESS is a voluntary Medicare program that supports ongoing care for certain chronic conditions. Participants work directly with an ACCESS organization like ETN Medical while keeping all standard Medicare benefits and freedom to see any Medicare provider.

You may be eligible if you:

• Have Original Medicare (Parts A and B)

• Are not enrolled in Medicare Advantage

• Have a qualifying chronic condition in one of our ACCESS tracks (confirmed during intake).

We will confirm eligibility before enrollment is complete. Submitting this form does not enroll you automatically.

Cardio-Kidney-Metabolic (CKM)

For heart, kidney, and metabolic conditions such as diabetes, chronic kidney disease, and high blood pressure.

Early CKM (eCKM)

For earlier-stage cardio-kidney-metabolic risk.

Behavioral Health (BH)

For depression and anxiety.

• Ongoing support from a physician-led team

• Care that continues between office visits

• Remote monitoring when appropriate

• Coordination with your primary care and referring doctors

• Regular check-ins to track progress on health goals

This complements but does not replace your usual Medicare care.

• Voluntary: You choose whether to participate.

• Original Medicare only at this time.

• Minimum participation: 90 days before you may switch ACCESS organizations.

• Cost-sharing: ETN Medical does not collect beneficiary cost-sharing for ACCESS services.

• Not for emergencies: Call 911 or go to the nearest emergency room.

• Privacy: We use your information only to respond to your request and provide care. See our Notice of Privacy Practices.

• No guarantee of acceptance: Eligibility is confirmed using Medicare and clinical criteria.

You may refer patients using our Refer a Patient Process or call (714 ) 432 - 8884. Referral is not required. Patients may sign up directly.
